For this type of request, we will need either an email, letter, form, or screenshot from the board’s website regarding the information that they need to verify. Some state boards also require that we fill out their own forms in addition to our letter. If this is the case for the specific state that is needing to verify your information please email the form to us at verifications@thomas-and-company.com . If the agency/board prefers to send it themselves, they may also email it to us directly. Please make sure that when you or the agency send us the request, your name, the last four digits of your social security number, the name of your employer, and the return information is provided with the request.
