Some state boards also require that we fill out their own forms in addition to our letter. If the agency/board prefers to send it themselves, they may also email it to us directly. Please make sure that when you or the agency send us the request, your name, the last four digits of your social security number, the name of your employer, and the return information is provided with the request.
