Ohio Identity Theft and 1099 Resources

If you suspect that your personal information has been stolen and/or if you received a 1099-G tax form from the Ohio Department of Job and Family Services (ODJFS) and did not apply for unemployment benefits last year, the agency needs to hear from you immediately.
